ABSTRACT

Objective@#To analyze the relationship between the toilet design and sanitary condition of primary and secondary schools in Chengdu and the epidemic situation of Norovirus infection, so as to provide scientific references for school toilet design and Norovirus prevention and control.@*Methods@#A total of 78 cases of norovirus epidemics from 2019 to 2020 were included in this study. These epidemics happened in 78 schools and the causes were indicated as human-to-human transmission. The Chi square and Mann-Whitney U test were used to compare categorical and continuous data respectively. Unconditional binary Logistic regression was used to examine the multivariate associations.@*Results@#Most epidemics happened from October in 2019 to March in 2020(79.49%, 62 cases), in primary schools (71.79%, 56 cases) and in the center area (52.56%, 41 cases). The median of the case number in each epidemic was 10. Overall, 56 schools (71.79%) were equipped with toilets which were flushed independently, and 22 schools (28.21%) were equipped with toilets which were flushed uniformly. There were 27 schools (34.62%) that did not have enough water taps. Logistic regression analysis found that those with toilets flushed uniformly had a stronger epidemic, compared to the schools with toilets flushed independently( OR=5.53, 95%CI=1.63-18.76, P <0.05).@*Conclusion@#In order to prevent intestinal infectious diseases , it is suggested that schools should design or reconstruct independent flushing toilets with adequate faucets.

ABSTRACT

Objective:To explore the regulatory effects of cow milk with the addition of breast milk equivalent dose of somatostatin (SST) and motilin (MTL) on food allergy and food intolerance. Methods:Young Brown Norway (BN) rats were divided into 5 groups and fed with pure breast milk(breast milk group), cow milk(cow milk group), cow milk added with SST(SST group), cow milk added with MTL(MTL group) and cow milk added with both SST+ MTL(SST+ MTL group). Allergic irritation was enhanced with skin smear at the same time.Clinical damages were quantified weekly.Levels of serous total Immunoglobulin E (IgE) and fecal calprotectin (FC) were detected by the enzyme-linked immunosorbent assay (ELISA). Gastric emptying ratio and intestinal propulsion ratio were measured by method of dextran blue. Results:In breast milk group, cow milk group, SST group, MTL group and SST + MTL group, the levels of IgE were (45.75±5.05) μg/L, (580.42±45.24) μg/L, (290.38±22.88) μg/L, (424.26±22.17) μg/L, (209.49±17.59) μg/L, respectively; FC level were (149.07±24.78) μg/g, (458.85±33.81) μg/g, (343.63±34.97) μg/g, (407.79±29.62) μg/g, (296.83±28.77) μg/g, respectively; the total score of clinical damage were (0.50±0.61) scores, (9.37±1.04) scores, (6.83±1.49) scores, (7.00±1.14) scores, (5.37±1.19) scores, respectively. The cow milk group had the highest scores of clinical damages.Compared with the cow milk group, the clinical damage score, IgE and FC of the SST, MTL and SST+ MTL groups had significantly lower levels, and there was significant difference among them (all P<0.01). The general status of the SST + MTL group was most similar to the breast milk group. The gastric emptying rate of MTL group was the closest to that of breast milk group [(92.52±6.27)% vs.(100.00±9.70)%, P<0.05]. There were obvious diarrhea and fast small intestinal propulsion in cow milk group, the small intestinal propulsion ratio in breast milk group was (39.32±2.61)%, and (71.96±4.43) % in cow milk group, the difference was statistically significant between the 2 groups ( P<0.01). The intestinal motility of SST+ MTL group was decreased, but it just prevented diarrhea caused by milk allergy, the small intestine propulsion ratio in SST+ MTL group was (38.90±2.65)% vs.breast milk group (39.32±2.61)%( P>0.05). Conclusions:The cow milk added with SST and MTL decreased allergic reaction and increased food tolerance in gastrointestinal tract, which was more similar to breast milk.SST was beneficial to relieving allergic immune reaction, MTL contributed to improving the gastrointestinal tolerance of cow milk. The combination of SST and MTL may achieve an antagonistic and balanced mechanism on gastrointestinal regulation, which could synergistically improve the gastrointestinal tolerance of cow milk.

ABSTRACT

Objective:To evaluate dynamic changes of functional status in elderly emergency inpatients by using the interRAI Acute Care for Comprehensive Geriatric Assessment(interRAI AC-CGA), and to analyze whether assessment results are helpful for clinical decision-making.Methods:Elderly patients(aged 60 years and older)who were hospitalized in the Acute Care Unit of PLA General Hospital from October 2017 to March 2018 were included. Each patient received the interRAI AC-CGA instrument-based assessment at admission(day 1), on day 7 and day 28 after admission.Analysis of collected data was conducted using interRAI AC-CGA scales, geriatric syndrome screening and risk identification of adverse outcomes, in order to examine dynamic changes of functional status in patients at and after admission and to explore the relationship between functional status at admission and outcomes(at discharge).Results:Thirty-four patients were evaluated, including 26 males and 8 females, aged from 60 to 101 years, with a mean age of(84.2±10.1)years.Patients had a variety of functional abnormalities at admission and during hospitalization, in which the ability to perform activities of daily living(ADL)and instrumental activities of daily living(IADL)decreased significantly.Compared with evaluation results on day 1, the incidences of functional abnormalities on day 7 and day 28 declined, and there was improvement in transient depression, pain and falls( χ2=6.298, 6.027 and 7.766, P=0.043, 0.049 and 0.021). After evaluation on day 28, 16 patients were discharged and 18 remained hospitalized(including 2 deaths). The rate for most abnormality was lower in the discharge group than in the continued hospitalization group, and the rates for abnormal communication and falls were lower in the discharge group than in the continued hospitalization group( χ2=6.349 and 5.443, P=0.019 and 0.030). Age(≥85 years old)had no effect on discharge. There was no significant difference in hospital discharge rate between the advanced age group(≥85 years old)and the elderly group(60-84 years old)(40.0% or 8/20 vs.57.1% or 8/14, χ2=0.971, P=0.324). Conclusions:The interRAI AC-CGA instrument can systematically and dynamically estimate functional changes in elderly emergency inpatients.Functional status at admission is related to prognosis(performance at discharge)of patients.

ABSTRACT

@#Carbon nanomaterials are one of the hotspots in the research of drug delivery systems for cancer. Compared with the traditional drug delivery systems for cancer treatment, carbon nanomaterials can be used as drug carriers after modification based on their advantages of large specific surface area and unique optical properties. They have the characteristics of high drug loading, good biocompatibility, tumor targeting and lasting action time, indicating great potential and development space. In this paper, the properties and functions of quantum dots, carbon nanotubes, graphene oxide and mesoporous carbon nanospheres are introduced in the order of dimensional quantum properties. The current research focus and existing problems of carbon nanomaterials are discussed in order to provide a reference for the safe and effective application of carbon nanomaterials in tumor therapy.

ABSTRACT

Pancreatic duct stone is a sequel of chronic pancreatitis and may be found in the main ducts, side branches or parenchyma. These stones obstruct the pancreatic ducts and produce ductal hypertension, which leads to pain, the cardinal feature of CP.Surgical operation has been the preferred treatment of pancreatic duct stones in many domestic and external pancreatic medical centers.Lithotomy by longitudinal pancreatic duct incision and Roux-en-Y anastomosis of pancreatic duct to jejunum is the main and effective surgical procedure, while micro-surgery was also rational for the treatment of pancreatic duct stones. However, further studies with a larger sample size and longer follow-up duration are needed to improve the surgical technique and verify our initial results.

ABSTRACT

As a member of PARP superfamily, PARP3 shares a high homology with PARP1 and PARP2, which are all DNA-dependent enzymes that are catalytically activated by DNA strand breaks.Compared to PARP1 and PARP2, PARP3 exerted some special properties in tissue expression pattern and biological function. The evidence has shown that PARP3 could be activated by DNA double strand breaks and special DNA single strand breaks and synthesize mono(ADP-ribose) (MAR) covalently attached to target proteins including itself.PARP3 plays an important role in DNA double strand breaks, DNA single strand breaks, activation of PARP1 and development of nervous system. It has been reported that PARP3 is associated with glioma and breast cancers. In this review, PARP3 structure, activation mechanism, biological function and its relationship with diseases will be presented.

ABSTRACT

Objective This paper explored the effect of the early clinical exposure in im-proving medical students' cognitive aspects of the doctor-patient relationship. Methods (1)From 280 undergraduate students of Grade 2010 who participated in early clinical exposure of Xi'an Jiaotong University College of Medicine and 280 undergraduate students of Grade 2011 who did not participate in the early clinical exposure, we selected 140 students respectively to conduct a simple random sam-pling survey. The results were analyzed by SPSS after using Microsoft Excellsoftware for entry. Statis-tical methods selected χ2-test/Fisher exact test(P<0.05). (2)We had an interview to students, teachers leading the project, instructor, experts engaging in the study of the doctor-patient relationship by using focus group discussion and in-depth interview, and then using thematic analysis to analyze the data. (3)The main aspects of questionnaires and interviews were: details, activity participation/interest, general cognitive on the doctor-patient relationship, passion for profession, choice of career, the effect in improving medical students' cognitive aspects of the doctor-patient relationship and the suggestions and comments. Results The effective questionnaires collected from Grade 2010 were 134, and124 from Grade 2011.The participation rate of the undergraduates of Grade 2010(participating group) was 94.0%(126/134), while the interest rate of undergraduates of Grade 2011 (control group) was only 76.6%(95/124). Participating group had a better cognitive understanding of doctor-patient relationship than control group(P=0.0192). The activities had significant effects on choice of career(P=0.0002), and no effect on passion for profession(P=0.7372). There was statistically significant difference on their views of employment(P=0.0002). The key words for the interview were: not enough preparation before activity, not reasonable timing, teachers leading the project, stimulation of professional pride. Conclusions Early clinical exposure activities can be effective in improving medical students' awareness of the current doctor-patient relationship. Still we have some shortages in the activity, some more exploration and amelioration should be made in late stage.

ABSTRACT

A sequential clean-up method was developed for the quantification of 10 plant growth regulators in bean sprout by the gas chromatography / mass spectrometry (GC / MS). The analytes were firstly extracted by the acided acetonitrile. Extraction was concentrated and re-dissovled by methanol. Then, it was divided to two aliquots. One of that was analyzed for 2,4-D-butyl ester and 2,4-D-ethyl ester after the purification by QuECHERS cartridge. Another one was treated by MCS solid phase extraction column including diverse eluting steps. After eluting by 5 mL methanol, composition 1 was obtain, concentrated, and methyl esterified by 10% boron trifluoride methanol solution. The treated extract was used for the determination of 4-chlorophenoxy acetic acid, β-naphthyl acetic acid, 2,4-dichlorophenoxy acetic acid, indole acetic acid and indole butyric acid. Composition 2 collected by eluting with 5 mL 5% amonium methanol was used for the determination of paclobutrazol, Kinetin, 6-Benzylaminopurine. The clean-up procedures are designed according to different chemistry properties of these plant growth regulators. The results showed that after spiking of 0. 01-0. 1 mg / kg selected plant growth regulators, average recovery ranged from 70. 0% to 93. 2%and relative standard deviation were 5. 2% -12. 3% . Limit of quantification (LOQ S / N≥10) and limit of detection (LOD S / N≥3) were 0. 01-0. 025 mg / kg and 0. 003-0. 008 mg / kg respectively. The developed purification method is easy, fast and accurate, and can be applied to routine test of plant growth regulators in bean sprout.

Objective To investigate into hand hygiene facilities and worker's practice of hand hygiene during 120 ambulance services in a capital city. Methods With the standard Observation Form publicized by the World Health Organization, field observation was carried out to monitor hand hygiene facilities and the practice of hand hygiene in workers during ambulance services provided by four service centers. Results A total of seven ambulances and 80 times of services were observed across a span of 21 days. The provision and position of hand hygiene facilities were found very limited in these ambulances.Of 656 times of presence of indictors for hand hygiene, the overall compliance rate was 3.04% reflecting the practice of hand hygiene in ambulance workers. The compliance rate was found to be 0 (0/291),1.96% (1/51),13.64% (3/22),5.21% (16/307) and 0(0/2) before touching patients, after body fluids exposure risk, before clean/aseptic procedures and after touching patients' surroundings respectively. Conclusions Poor practice of hand hygiene which was revealed in 120 ambulance services suggests the great need for education and training to improve patient safety and occupational health.

ABSTRACT

Objective To study the expression of homocysteine in serum of patients with common malignant tumor, and initially investigate the possibility of serum homocysteine as cancer biomarker. Methods Expression levels of homocysteine and caner biomarkers (CEA,AFP,CA125,CA199) in serum of 180 patients with established malignant tumor and 30 healthy controls (control) were measured, the results of homocysteine were compared with that of the cancer biomarkers based on the cutoff value used in clinic. Results The expression levels of homocysteine was significantly higher in patients with malignant tumor than in controls [(13.89 ± 4.95) μmol/L-(21.40 ± 9.38) μ mol/L vs (11.40 ± 3.13) μmol/L,P < 0.05)].Conclusions The positive predictive rate of homocysteine is higher than the four kind of cancer biomarkers in lung cancer, breast cancer, esophageal cancer. The increase of homocysteine in tumors may be universal, and Homocysteine may be used as cancer biomarker in lung cancer, breast cancer and esophageal cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the behavioral risk of toxoplasma infection in special population.</p><p><b>METHODS</b>Serum samples from workers in slaughterhouse were collected and tested for the detection of cAg, toxo-IgG, toxo-IgM antibody to toxoplasma, using ELISA. Behavioral measurements were conducted through questionnaire, which was developed according to theory of reasoned action which consists of a measurement model that specifies the relation of measured to behavioral variables and a behavioral variable model to show the influence of behavior variables on each other. Factor analysis methods were used for estimating the parameter of the former and path analyses method for the latter.</p><p><b>RESULTS</b>With data from 302 research subjects, the responding rate was 95.87%. The overall infection rate was of 19.20% including 15.89% having cAg, 5.63% with toxo-IgG, 2.32% with toxo-IgM, 1.32% with the third of cAg, toxo-IgG and toxo-IgM, 2.65% with the both cAg and toxo-IgG, 1.99% with the both cAg and IgM respectively. There was significant association between toxoplasma infections and contact with dystocia or live pigs, body fluid or harslet. There was no relation between toxoplasma infections and abortion.</p><p><b>RESULTS</b>from miscarriage factor analyses showed that except evaluation of behavioral outcomes and behavioral attitudes, internal consistence reliability of others behavioral variables was good (Cronbach's alpha > 0.7). These behavior variables could be explained by their measure items. Path analyses suggested that two components had provided significant independent contributions to those preventive behaviors which significantly correlated to toxoplama infections. Half of the contribution leading to toxoplasma infections had come from behavioral factors. Belief was relatively more important in predicting the preventive behavior than subjective norm.</p><p><b>CONCLUSION</b>Our findings supported the application of reasoned action theory in understanding the role of behavioral factors in toxoplasma infection.</p>
